Condition for waiving fees (condition 5.1.1)

A request for refund of fees for illness or injury must use this form and be returned to the address provided no later than 18:00hrs on the Thursday before the competition date. Late requests will not be considered.

Submission of the this form does not constitute a withdrawal which must be completed separately.

All applications will be reviewed after the close of the Championships.
