County Team Championships 2025

You have been asked to complete this form because your swimmer has been invited to join the team to represent Sussex at the County Team Championships 2025 taking place at Ponds Forge, Sheffield.

The Team travel together to Sheffield with a training session in Loughborough on Saturday 4th October, stay overnight in a central hotel and then compete on the Sunday 5th before travelling home.

Parents are of course very welcomed to support the team in Sheffield but we cannot provide transport or arrange accommodation.

Please return the completed form by Friday 12th September.

This form must be completed by a parent or guardian of each swimmer selected as part of the Sussex Squad.

By completing the form you are also giving permission for the Team Manager to give the immediate necessary authority on your behalf for any medical or surgical treatment recommended by competent medical authorities, where it would be contrary to your son/daughter’s interest, in the doctor’s medical opinion, for any delay to be incurred by seeking your personal consent. 

This sections asks for information about your swimmer's kit requirements.

Every squad member is provided with a polo shirt and hoodie as well as other personal items.

If your swimmer was a member of the team in 2024 and their hoodie and polo shirt are still useable please consider using them again. If they are not useable please complete the appropriate section below.

If this is the first time that your swimmer has been selected to swim for Sussex please order the size of polo shirt and hoodie they need. (kit will be distributed throughout the trip)

This section asks about the swimmer's menu selections.

The squad will be eating together at Zizzi in Leopold Square on the evening of Saturday 4th October. Additional salads, breads and soft drinks will provided on the table. If you need additional information about the menu please use the link above.

Zizzi have an extensive menu with options for allergens. Please use the box below to tell us about any special dietary requirements so that we can discuss these with the restaurant.

The section is about travelling to and from Sheffield.

Please tell us where you want to meet the coach travelling to Sheffield on Saturday 4th October and where you want to meet the coach on its return trip on Sunday night.

It is essential that you are at the pick up point on time.

We will ask swimmers to phone ahead when we are close to drop off points on the return journey.
