This form must be completed by a parent or guardian of each swimmer selected as part of the Sussex County ASA Squad attending the National County Team Championships. By completing the form you are also giving permission for the Team Manager to give the immediate necessary authority on your behalf for any medical or surgical treatment recommended by competent medical authorities, where it would be contrary to your son/daughter’s interest, in the doctor’s medical opinion, for any delay to be incurred by seeking your personal consent. Please answer all the questions on this form and submit it by midday on Wednesday 20th September.
